New Patient Form

We request new clients to Yorktown Animal Hospital fill out the new patient form for our records. This form may be completed by filling out the fields below. You can also download and print a PDF version* and bring it with you to your pet’s first appointment.

For the safety of our staff and the benefit of your pet, employees of Yorktown Animal Hospital are unable to perform walk-in services on your pet requiring physical interaction if your pet does NOT have a current Rabies vaccination on file. This includes but is not limited to pedicures, Bordatella vaccinations, and Heartworm and Fecal Exams.

The only exceptions to this rule:

Animals too young for Rabies vaccination (under 4 months)

Animals that have a documented autoimmune disease or vaccine reaction

COVID-19 Protocols

We are limiting how many new clients we are scheduling appointments for, availability may vary and may be 2-3 weeks before we can see your pet for routine examinations.

We will also be requiring a $60 deposit for New Clients that want 2 animals to be seen in 1 appointment ($60 deposit will be applied to the appointment invoice, will be refunded if the appointment is canceled 48 hours prior)
